How early should patients arrive to their first appointment at Two Wells Family Medical Centre?

Patients should arrive 15 minutes early to their first appointment in order to ensure all their details are correct and can be changed if needed.

 

Can patients transfer their medical records from another practice?

We can organise to have your medical records transferred from another practice. Please see our reception staff for more information. Patients will need to sign a release of information consent form at reception.

 

Can patients transfer their medical records to another practice?

We understand that for any number of reasons, patients may need to transfer their care to another practice.

In order to provide your new practice with your medical records, we request a written authority from the receiving clinic, signed by patients. We will promptly attend to such requests by providing a summary of your medical records directly to the new practice. If your new practice requires a copy of your entire medical record to be sent, please note that an administration fee may apply to such requests, and in addition to this, retrieval fees may also apply for some patients whose medical history is archived.

If you wish to have your medical records transferred from another clinic to Two Wells Family Medical Centre, please present in clinic to fill out a Request for Transfer of Records form.

 

What is the after hours policy?

For appointments out of hours, please call the Hello Home Doctors on 13 41 00 or the Health Direct helpline for advice on 1800 022 222.
In an emergency, please call Triple Zero (000) or go to your nearest hospital.

 

Do you accept walk-ins?

Yes, we do accept walk-ins.

 

Do patients need an appointment for a specialist referral/medical certificate/repeat prescriptions?

Yes, appointments are required for each of these services.

 

Is parking available?

Yes, parking is available on site.
